Patient Name:

Discharge Date:

Cardiac Stress Test to be completed by:

Your discharge is partially dependent on you completing a diagnostic cardiac stress test. Your condition is stable at this time; however, it is important for you to follow-up with a cardiologist for further evaluation.

This stress testing should be performed within 72 hours of discharge from the Emergency Department.

You will receive a call from Methodist scheduling department regarding the time of cardiac stress testing and follow-up appointment.

To ensure that we are able to perform your stress test in a timely manner, please make sure that you have spoken with someone from the Methodist scheduling department and received an appointment time before arriving on your test date.

If you do not have a cardiologist, Methodist will assign an in-network cardiologist to your case, based on your insurance status and coverage.

It is important that you receive this diagnostic test in order to complete your evaluation. If you do not receive a call from Methodist scheduling department by the date of the stress test above regarding your required follow-up care, please call our office immediately at 901-763-0200 and ask for the LRED Coordinator.
